Atlas Direct can be part of your efforts to rein in healthcare spending while continuing to provide health benefits that are both highly competitive and fully compliant.
Atlas Direct is an association health plan sponsored by the Direct Care Foundation. Individuals—including you and your employees—must join the Direct Care Foundation to be eligible to apply for Atlas Direct insurance coverage, which requires underwriting in all the states where it is available.
If approved for coverage, monthly Atlas Direct insurance premiums may be paid by either the employee or the employer, thereby giving employers the opportunity to cover the cost of Atlas Direct on behalf of their employees.
Whether you're the owner of a small business or the CEO of a major company, Atlas Direct gives you access to a supplemental insurance product that was specifically designed to leverage the cost savings and quality improvements generated by Direct Primary Care.
Atlas Direct is designed to work equally well with either employer-paid or employee-paid Direct Primary Care—thereby giving you more options with respect to obtaining the health benefits and costs savings generated by Direct Primary Care.
Each covered person is charged a monthly premium based on age and sex, with the total premium for multiple individuals equal to the sum of their individual premiums.
The table below shows how much Atlas Direct’s insurance plan costs per person per month in most states. These prices are for Atlas Direct coverage only. DPC services must be purchased separately from a Direct Primary Care doctor or clinic.
The table below show’s Atlas Direct’s monthly premiums per covered person.
Premiums may vary by state, are based on attained age (age at last birthday), and will increase when you move into the next-higher age range. Atlas Direct is not available in all states. Please click here to get a custom quote and see whether Atlas Direct is available in your state.
Individuals can join the Direct Care Foundation and apply for Atlas Direct any day of the year. If approved, they can start their coverage as soon as the first day of the upcoming month. There is no need for them to wait for an annual open-enrollment period or for their employer to redesign an entire health-benefits package. This flexibility may be particularly helpful for employees searching for a supplemental insurance product to help cover the deductibles and copays of a major medical plan.
Apply TodayAs a limited-benefit supplemental insurance product, Atlas Direct costs substantially less per individual or family than comprehensive major medical coverage.

The Omnibus Budget Reconciliation Act of 2026 revised the rules governing health savings accounts (HSAs) in ways that allow them to be used to pay for direct primary care (DPC) monthly membership fees. The new rules still impose important limitations on the use of HSA funds and require regulations or guidance to be issued in the future. The premiums paid for the Atlas Direct insurance program do not qualify as an eligible HSA expense.
